1. Acceptance of Terms
These Terms of Service govern your use of tulsasculptedsmiles.com and any dental services provided by Tulsa Sculpted Smiles, operated by Dr. Wade Sessom, DDS, located at 4415 S Harvard Ave, Tulsa, OK 74135.
By visiting our website, submitting a contact form, scheduling an appointment, or receiving care at our practice, you confirm that you have read, understood, and agreed to these Terms. If you are scheduling on behalf of a minor patient, you confirm you are the parent or legal guardian and accept these Terms on their behalf.
2. Dental Services
Tulsa Sculpted Smiles provides family and general dentistry services including preventive care, teeth cleanings, Invisalign, cosmetic dentistry, tooth replacement, and other treatments as described on our Services page. All treatment is subject to a clinical examination, accurate patient health history, your informed consent, and clinical suitability as determined by your provider. We reserve the right to decline treatment at our clinical discretion or where doing so would be inconsistent with the standard of care.
3. Appointments & Cancellations
Appointments may be requested online, by phone, or through our booking portal. Please notify our office at least 24 hours in advance if you need to cancel or reschedule. Repeated missed appointments or late cancellations without notice may result in a cancellation fee or a requirement to prepay future visits. New patients should arrive 10–15 minutes early to complete required paperwork. Late arrivals may result in a shortened appointment or rescheduling at our discretion.
4. Payment & Financial Policies
We are in-network with many insurance plans including Aetna, Cigna, and Guardian. You are responsible for verifying your coverage prior to your appointment and for any balance not covered by your insurer. Payment for services not covered by insurance is due at the time of your visit. We accept major credit cards, cash, and online payment. Financing is available through CareCredit and Alphaeon Credit, subject to lender approval. Cost estimates are good-faith approximations and may vary based on clinical findings.
5. Website Use
All content on this Site — including text, images, and logos — is owned by or licensed to Tulsa Sculpted Smiles. You may not reproduce or distribute any content without our written permission. You agree not to use the Site for unlawful purposes, attempt to gain unauthorized access, or use automated tools to scrape content from the Site.
6. Electronic Communications
By providing your phone number or email address, you consent to receiving appointment confirmations, reminders, and general communications from our practice electronically. You may opt out of marketing emails at any time. Text STOP to opt out of text-based reminders. Standard carrier messaging rates may apply.
7. Limitation of Liability
To the fullest extent permitted by Oklahoma law, Tulsa Sculpted Smiles, Dr. Wade Sessom, and our staff shall not be liable for any indirect, incidental, or consequential damages arising from your use of this Site or reliance on its content.
